1. Admission forms for Master of Physiotherapy 2018-19 for NRI/ Institutional quota and Vacancy quota are available on this website. Please download the form.
2. Before filling up the application form the applicant must carefully go through the instructions given in advertisement / information booklet.
3. Candidate passing in BPTh and completing his/her six months compulsory internship on or before 31st August 2018 will be eligible and PGP-CET 2018 is necessary.
4. Application form must be written in candidate's own handwriting in BLOCK LETTERS with clear writing on the heading as ‘INSTITUTIONAL QUOTA’ or ‘NRI QUOTA’ or ‘VACANCY QUOTA’
5. Incomplete application form or forms without attested true copies of certificates (mentioned in the form) will be summarily rejected.
6. Applications form completed in all respects should be submitted along with Cash in the office of college within the stipulated period. Delay in submission of application may be condoned in very exceptional case.
7. Admissions will be as per merit of PGP-CET 2018 marks (Institutional/Vacancy Quota). Applicants are required to pursue the progress of selection process by diligently observing notifications by the Principal as displayed on Notice Board. No individual letters will be sent to the applicants nor any correspondence in this concern be entertained.
8. Applicant must specify if any of his original certificates is bonafide DUPLICATE certificate issued by the Competent Authority.
9. Visa type size photograph must be pasted in the space provided for.
10. Once the application form is submitted in the college office it will not be returned for any corrections
